Umbrella Hub Arrangement Spotlights

Umbrella hub arrangements (UHAs) connect community-based organizations (CBOs) with health care payment systems to pursue sustainable reimbursement for the National Diabetes Prevention Program (National DPP) lifestyle change program. Each UHA is administered by an umbrella hub organization (UHO), which is the lead organization in the UHA. This page showcases the experiences of several organizations that have undertaken the UHO work. The UHOs, the subsidiary organizations they work with, their business models, the obstacles they encountered, and the paths they take to developing UHAs are diverse. The Spotlights provide details about each UHO, their areas of focus, and provide specifics about their UHA development. The approaches these UHOs took to operationalize their UHAs may be instructive to other UHOs, to organizations that might become UHOs, as well as to partners that support UHA development (see the UHA Roadmap page of the Coverage Toolkit for more information on UHA operationalization).
