A Roadmap for States to Implement the National DPP Lifestyle Change Program as a Medicaid Benefit

 

The Policy to Payment Roadmap below contains activities that lead to coverage and operationalization of the National Diabetes Prevention Program (National DPP) lifestyle change program. The intended audiences for this resource are state Medicaid agencies and state public health departments. The roadmap is also intended to serve as an informational resource for CDC-recognized organizations, managed care organizations (MCOs), and health care providers.

States have flexibility in how to approach seeking and operationalizing coverage of the National DPP lifestyle change program. The Policy to Payment Roadmap captures many of the steps pursued by states that have achieved Medicaid coverage of the program. However, each state journey is unique, including the order in which the steps occur.
