About Us

The National Association of Chronic Disease Directors (NACDD) and the Division of Diabetes Translation at the Centers for Disease Control and Prevention (CDC) created the National Diabetes Prevention Program Coverage Toolkit in 2017 as part of the Medicaid Coverage for the National Diabetes Prevention Program (National DPP) Demonstration Project. The demonstration project was established in 2015 and ended in January 2019. NACDD contracted with Leavitt Partners, an HMA Company, to take lead on developing this site with input from multiple health plans, national partners, state Medicaid agencies, state public health departments, CDC, and NACDD.

This Coverage Toolkit is designed to be a national resource for organizations seeking to expand access to the National DPP lifestyle change program through sustainable health care coverage and payment strategies. This Toolkit provides practical guidance, tools, and resources for:

  • State public health departments
  • State Medicaid agencies
  • Managed care organizations (MCOs)
  • Employers
  • Commercial health plans
  • Medicare Advantage (MA) plans
  • MDPP suppliers
  • Community-based organizations, and
  • Other partners interested in implementing and sustaining coverage for the program.

Want to learn more about how the toolkit was developed? NACDD has created a one-page infographic to describe how the toolkit was built and disseminated from its beginning.
