About Us
The National Association of Chronic Disease Directors (NACDD) and the Division of Diabetes Translation at the Centers for Disease Control and Prevention (CDC) created the National Diabetes Prevention Program Coverage Toolkit in 2017 as part of the Medicaid Coverage for the National Diabetes Prevention Program (National DPP) Demonstration Project. The demonstration project was established in 2015 and ended in January 2019. NACDD contracted with Leavitt Partners, LLC to take lead on developing this site with input from multiple health plans, national partners, state Medicaid agencies, state public health departments, CDC, and NACDD.
We envision this toolkit will serve as a national resource for states that want to explore, move forward with, and sustain Medicaid coverage for the National DPP lifestyle change program. It also provides information and resources for employers and commercial health plans interested in covering the program.

The National Diabetes Prevention Program and the Division of Diabetes Translation at CDC
The Division of Diabetes Translation is a part of the National Center for Chronic Disease Prevention and Health Promotion at CDC. The Division established and oversees the National Diabetes Prevention Program – a public-private initiative to prevent type 2 diabetes by offering evidence-based, cost effective interventions in communities across the US.
The mark “CDC” is owned by the US Dept. of Health and Human Services and is used with permission. Use of this logo is not an endorsement by HHS or CDC of any particular product, service, or enterprise.
National Association of Chronic Disease Directors (NACDD)
The National Association of Chronic Disease Directors (NACDD) is a nonprofit, public health organization for chronic disease program directors of each state and U.S. territory. Founded in 1988, NACDD connects more than 6,500 members to advocate for preventive policies and programs, encourage knowledge sharing, and develop partnerships for health promotion. Since its founding, NACDD has been a national leader in mobilizing efforts to reduce chronic diseases and their associated risk factors through state and community-based prevention strategies.

Expert Panel
An Expert Panel comprised of government, professional associations, and business organizations (some of which are represented below) was convened in December 2015 to provide fundamental guidance for the development of the Medicaid Coverage for the National DPP Demonstration Project as well as ongoing expertise throughout the project. Individuals and organizations on the Expert Panel were selected based on their familiarity with the National Diabetes Prevention Program as well as their significant leadership in the changing Medicaid landscape.

Association for Community Affiliated Plans
The Association for Community Affiliated Plans (ACAP) is a national trade association that represents 60 not-for-profit Safety Net Health Plans. Collectively, ACAP plans serve more than seventeen million enrollees, representing nearly half of all individuals enrolled in Medicaid managed care plans. ACAP’s mission is to strengthen not-for-profit Safety Net Health Plans in their work to improve the health of lower-income and vulnerable populations.
